INtake process
Anything that is shared between us, stays between us.
Congratulations, the first step towards change is the hardest. You have made the first step, and we are here to support you and your whanau to make long lasting lifestyle changes.
To make an appointment we need to know who you are. This can be achieved in a few ways. Firstly, you can use this website, click on the contact link in the heading and send us a message. This will go into our email system and we will respond to you. Alternatively you can email Jayda@Maraeroa.Maori.NZ. Both ways will result in an email being sent to us. Secondly, you can call us on (04) 235 8000 ext 109 or 027 529 3215 and ask to speak to Jayda. Thirdly you can ask another organisation to refer you to us.
On the day of your appointment, please feel free to bring support people with you. This is your choice, some people find it helpful.
At your first visit with us we will collect some information that will help us design a service that is individualized to your needs. Everyone is different. Your needs are not the same as my needs, and we both have different end goals. Consequently, the service that we provide is a reflection of the life changes you intend to make. We are here to support you to make those changes. At this time, we will also be able to book in a time for you to meet with the AoD Clinician. If you require a health check, we are able to make a time for you to meet with one of our in-house nurses.
When you meet with the AoD Clinician, all of the information shared remains confidential. Together, you and the AoD Clinician will choose a path towards change that is achievable. Further bookings will be made with the AoD Clinician as you need them.
